SCOPE OF WORK CITY OF PINEHURST, TEXAS CDBG-DR Contract No. 20-065-034-C113 STREET IMPROVEMENTS GENERAL This contract provides for asphalt street rehabilitation of multiple streets within the City of Pinehurst. SPECIFIC This contract shall include all necessary labor, materials, equipment, tools, inspection, testing, and superintendence required for the complete and operable rehabilitation of the proposed project as shown on the contract plans and specifications. This contract includes, but is not limited to the following: Base Bid Items, Category 1 Streets. Contractor shall rehabilitate approximately 24,919 linear feet of asphalt roadway on various streets by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime per TxDOT Item 260, installing Mirafi MP400 geotextile fabric, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. Category 2 Streets. Contractor shall rehabilitate approximately 13,415 linear feet of asphalt roadway on various streets by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime-flyash per TxDOT Item 265, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. West Park Avenue. Contractor shall rehabilitate approximately 4,860 linear feet of concrete roadway by milling the existing asphalt surface, providing full depth repair of the concrete pavement at the locations shown on the plans, applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. West Park Avenue. Contractor shall furnish and install approximately 1,920 linear feet of double 4” continuous TYPE I (Y) pavement marking per TxDOT Item 666, all in strict accordance with the plans and specifications. West Park Avenue. Contractor shall furnish and install approximately 2,940 linear feet of 4” dashed TYPE I (Y) pavement marking per TxDOT Item 666, all in strict accordance with the plans and specifications. Additive Bid Items, South 30th Street. Contractor shall rehabilitate approximately 528 linear feet of asphalt roadway by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime per TxDOT Item 260, installing Mirafi MP400 geotextile fabric, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. South 29th Street. Contractor shall rehabilitate approximately 680 linear feet of asphalt roadway by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime-flyash per TxDOT Item 265, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. Harding Street. Contractor shall rehabilitate approximately 2,621 linear feet of asphalt roadway by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime per TxDOT Item 260, installing Mirafi MP400 geotextile fabric, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. Worley Street. Contractor shall rehabilitate approximately 163 linear feet of asphalt roadway by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime-flyash per TxDOT Item 265, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. Elma Street. Contractor shall rehabilitate approximately 833 linear feet of asphalt roadway by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime per TxDOT Item 260, installing Mirafi MP400 geotextile fabric, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. North 30th Street. Contractor shall rehabilitate approximately 721 linear feet of asphalt roadway by pulvamixing the existing roadway to a depth of twelve (12”) inches, mixing in lime per TxDOT Item 260, installing Mirafi MP400 geotextile fabric, and applying 2” of Type “D” HMAC surfacing per TxDOT Item 340, including driveway and street transitions, removing and resetting mailboxes (as required) and traffic control, all in strict accordance with the plans and specifications. West park Avenue. Contractor shall furnish and install approximately 8,000 linear feet of 4” continuous TYPE I (W) pavement marking per TxDOT Item 666, all in strict accordance with the plans and specifications. CONDITIONS ALONG THE ROUTES The Contractor's attention is called to various physical conditions along the routes as follows. The Contractor shall take precautions to avoid damage to power lines, foundations, guy wires, and appurtenances. The Contractor shall observe all applicable OSHA and owner requirements while working near the overhead power lines including restriction of height of equipment.During construction, the Contractor shall take the necessary precautions to avoid damaging existing utilities within the limits of the project site. SPECIAL REQUIREMENTS AND CLARIFICATIONS The Contractor shall restore the work route to its original condition or better prior to acceptance by the Owner. The Contractor shall contact the Mr. John Tony at (337) 404-4377 prior to starting any work within the project area. Any changes in quantities will be addressed by change order and will have to be approved by the Owner before any work can be done.
